(carrying on business as “Bungee Workout Canada” and referred to as Bungee Workout Canada) makes every effort to ensure your personal information is protected while it is in our custody. Collection, Use and Disclosure of Personal Information We collect the following personal information on the registration form: name, address, date of birth, e-mail address, business and home phone numbers, credit card number, and the like. We may also have information about your health and fitness goals, if you have communicated these to us. We collect this information to record payment and attendance records, to note injuries or individual fitness needs, to notify our clients of promotions or schedule changes, to keep in touch with lapsed clients and to provide on-going service. We may use this information to compile internal marketing information. We collect the information from individual class sign-up sheets for marketing, to monitor class sizes, and for emergencies. We collect credit card information to use as payment from clients. We will not collect, use or disclose any personal information for any other purposes without explicit consent except where PIPA allows. When you become a client of Bungee Workout Canada, you consent to the use of your personal information for the purposes listed above. You may refuse or withdraw your consent at any time; however we may not then be able to provide you with our full range of services.
